Whistle Blower Policy

Purpose

To provide clients, family members, advocates, community members, volunteers, and contractors with a safe, confidential, and transparent way to report serious wrongdoing or misconduct within Your Lifestyle Centre (YLC).

Scope

This policy applies to anyone who interacts with YLC’s services and wishes to raise concerns about suspected misconduct, abuse, neglect, fraud, or breaches of the Aged Care Act 2024 or Aged Care Quality Standards.

Who Can Report

Clients, family members, carers, community members, volunteers, suppliers, or contractors.

What Can Be Reported

Serious wrongdoing such as financial misconduct, fraud, corruption, neglect, abuse, health and safety risks, or breaches of governance or care standards.

How to Report

Concerns can be raised confidentially or anonymously by emailing governance@ylc.org.au, president@ylc.org.au or completing the send us feedback on our webpage.  Information can also be sent directly to PO Box 14 Beenleigh Qld 4207 or handing placed in our feedback box at 20 Alamein St Beenleigh located in our reception.

Protections

All reports are treated confidentially. YLC prohibits any victimisation or retaliation against a person making a report in good faith.

Investigation Process

Reports will be acknowledged, assessed, and investigated by the CEO or an independent delegate. Matters may be escalated to the Clinical Specialist Board Member where appropriate.

External Reporting

Whistleblowers may also report directly to the Aged Care Quality and Safety Commission,  Australian Federal Police (for criminal matters), or other relevant authorities.

Record Keeping

Reports and actions will be logged within in our internal recording system and reported through to Department of Health and Aging (if applicable) and monitored for continuous improvement.
