How to Share Your Feedback or Make a Complaint
You can tell us:
• In person – speak to any member of our staff
• By phone or email – contact our office on 07 32871477 or email feedback@ylc.org.au
• In writing – complete a ‘Tell Us What You Think’ form available at reception or from staff
- On our webpage – Send us Feedback
• Through your care worker or manager
• By asking a family member, carer, or advocate to contact us on your behalf. You can choose to give feedback anonymously if you prefer.
What Happens Next
- Your feedback or complaint will be acknowledged quickly.
• We will review the matter and discuss possible solutions with you.
• You will be kept informed throughout the process.
• Once resolved, we will confirm the outcome and how improvements will be made.
• All feedback and complaints are handled confidentially, fairly, and without judgement or retribution.

You have the right to use an advocate (someone who can speak on your behalf). You can choose a friend, family member, or contact:
